Local Tax Collector for wrong municipality?

I recently received a 2010 Earned Income Tax Return Form from the Municipal Tax Collector for Schuykill Township which belongs to Chester County. However, I am a resident of Upper Providence Township which belongs to Montgomery County. I am at lost what to do at this point and I am not sure who to contact. Any advise would be greatly appreciated. Thank you.
